When a muscle remains tight and painful, simply resting may not be enough.

The painful area may feel hard or knotted. Movement may become limited, and the surrounding muscles may begin to tighten as well. Over time, a cycle can develop:


Pain causes the body to guard the area, the muscles remain tense, and that tension makes movement more uncomfortable.


Acupuncture is not simply a matter of placing needles wherever it hurts. A thin acupuncture needle creates a controlled mechanical and sensory stimulus in the skin, connective tissue, and muscle. Research suggests that this stimulation may influence local tissue responses, pain-signaling nerves, and the nervous system’s own pain-regulating pathways. These different effects may work together to reduce pain and help movement feel more comfortable. (PubMed)




Acupuncture Creates a Physical Response in the Tissue


An acupuncture needle is very thin, but the body does not simply ignore it.

When a needle enters the tissue and is gently stimulated, it interacts with the connective tissue surrounding the muscle. Experimental research has shown that needle manipulation can wind collagen around the needle and cause nearby connective-tissue cells called fibroblasts to change their shape and internal structure.

This process is known as mechanotransduction, which means that a physical stimulus is converted into a biological response inside the tissue. The full clinical significance is still being studied, but this research helps explain why acupuncture is more than simply touching the surface of a painful area. (PubMed)

For a patient with shoulder pain, neck pain, or lower-back pain, the goal is not to force the muscle to relax. The goal is to give the painful and guarded area an appropriate stimulus, then monitor whether the muscle feels less tense, movement improves, and tenderness decreases.



Acupuncture May Reduce Local Pain Signaling


Acupuncture stimulation may also affect chemical signaling around the treatment area.

In laboratory research, acupuncture increased the local release of adenosine, a naturally occurring substance involved in regulating nerve activity and pain sensitivity. Activation of adenosine A1 receptors reduced pain-related responses in animal models of inflammatory and nerve pain. (Nature)

This does not mean that one chemical explains every acupuncture response. Pain is more complicated than that. However, it provides a biological explanation for how stimulation at the treatment site may reduce the amount of pain information being transmitted through nearby sensory nerves.

This may be particularly relevant when a muscle is tender to touch, painful with movement, or caught in a pattern of repeated tightening.



Acupuncture Communicates With the Nervous System


Muscle pain is felt in the muscle, but pain is ultimately processed by the nervous system.

When an acupuncture needle stimulates sensory nerves in the skin and muscle, signals travel through the spinal cord and brain. Human imaging studies suggest that acupuncture may influence regions involved in pain processing and the brain’s descending pain-modulation system.

In a study of patients with chronic lower-back pain, four weeks of real acupuncture produced greater improvement in pain bothersomeness than sham acupuncture and changed communication between brain regions involved in pain control, emotion, and motivation. The amount of change in these networks was associated with the reduction in pain bothersomeness. (PubMed)

Human PET studies have also found that real acupuncture and sham acupuncture can affect the brain differently, including differences involving the body’s mu-opioid receptor system, which participates in natural pain regulation. (PubMed)

In simpler terms, acupuncture may help the nervous system turn down an overly active pain signal instead of only focusing on the painful muscle itself.



Why Can a Painful Muscle Stay Tight?


When movement hurts, the body naturally tries to protect the area.

A patient with shoulder pain may avoid lifting the arm. Someone with lower-back pain may move stiffly or rely more heavily on other muscles. A patient with neck pain may stop turning the head fully while driving. These protective movements can be useful at first, but when they continue, the muscles may remain guarded and normal movement can become more difficult.

Acupuncture may help interrupt this pain-tension-pain cycle by reducing pain sensitivity and giving the patient an opportunity to move the area more comfortably.

That is why we do not evaluate progress only by asking whether the pain is completely gone. We also look for practical changes:

  • Can the neck turn more comfortably?

  • Can the shoulder lift higher?

  • Can the patient sit or walk longer with less lower-back pain?

  • Is it easier to stand up from a chair?

  • Is the patient waking less often because of pain?

  • Does the muscle feel less tight or tender?

These changes often matter just as much as the pain number.



Acupuncture for Neck and Shoulder Pain


Neck pain and shoulder pain commonly occur together.

Muscles around the neck, upper back, and shoulder blade may become tight after prolonged computer work, repetitive arm use, driving, physical labor, stress, a previous injury, or an uncomfortable sleeping position.

Patients often ask about acupuncture when they experience:

  • Tight neck and shoulder muscles

  • Pain when turning the head

  • Shoulder pain when lifting or reaching

  • Upper-back tension around the shoulder blades

  • Pain that becomes worse after work or driving

  • Nighttime shoulder pain

  • Recurring muscle knots and tenderness

  • Limited neck or shoulder movement

During treatment, we look at where the pain is located, but we also check the muscles and movements surrounding that area. Shoulder pain can sometimes be related to the neck, and neck pain can be influenced by the upper-back and shoulder muscles.

Treating only the exact spot that hurts may miss part of the problem.



Acupuncture for Lower-Back Pain


Lower-back pain is another common reason patients seek acupuncture in Koreatown, Los Angeles.

Back muscles may tighten after lifting, repetitive work, prolonged sitting, driving, gardening, exercise, a fall, or a previous injury. In some patients, there is no single cause. The pain may gradually develop after years of physical strain.

Acupuncture may be considered for:

  • Chronic lower-back pain

  • Lower-back muscle tension

  • Pain after lifting or bending

  • Back pain aggravated by sitting or driving

  • Pain that becomes worse while walking

  • Recurring back-pain flare-ups

  • Stiffness when standing from a chair

  • Lower-back pain that affects sleep or daily activities

A large individual-patient-data meta-analysis included 39 randomized trials and 20,827 patients with chronic musculoskeletal pain, osteoarthritis, headache, or shoulder pain. Acupuncture produced better outcomes than both sham acupuncture and no-acupuncture controls, and the authors concluded that referral for acupuncture is a reasonable option for chronic pain. (CORE)

This does not mean that all back pain is muscular. Pain accompanied by significant weakness, rapidly worsening numbness, bowel or bladder changes, fever, or major trauma requires prompt medical evaluation.



Acupuncture for Knee Pain and Other Musculoskeletal Pain


Muscles also play an important role in knee pain.

When the knee hurts, the muscles around the hip, thigh, and lower leg may tighten or stop working normally. This can make walking, climbing stairs, and standing from a chair more difficult.

Patients may inquire about acupuncture for:

  • Knee pain while walking

  • Knee stiffness

  • Pain when using stairs

  • Pain when standing from a seated position

  • Tight muscles around the knee

  • Osteoarthritis-related discomfort

  • Persistent pain after a previous injury

Acupuncture does not rebuild damaged cartilage or replace an orthopedic evaluation when one is necessary. However, it may be used as part of a broader plan to manage pain, muscle tightness, movement, and daily function.



What Happens During the First Visit?


The first visit begins by understanding what the pain is actually doing to your daily life.

We ask:

  • When did the pain begin?

  • What activity or movement makes it worse?

  • Does the pain interfere with sleep?

  • Is there stiffness, numbness, tingling, or weakness?

  • How long can you sit, stand, or walk comfortably?

  • Have you received imaging, injections, surgery, physical therapy, or other treatment?

  • Which activities would you like to return to?

We may also check tenderness, muscle tension, range of motion, pain with movement, and differences between the right and left sides.

Treatment is then planned based on the consultation and evaluation. The location and number of needles are not identical for every patient because every case of neck pain, shoulder pain, lower-back pain, and knee pain is different.



When Acupuncture Starts Helping


Some patients first notice that the painful muscle feels softer or less heavy. Others notice that they can turn, reach, walk, or sit more comfortably. Sleep may improve when the painful area is no longer waking them as often.

The goal is not to create a temporary sensation and assume the problem is solved. We look for changes that the patient can actually feel in daily life.

For example:

  • Shoulder pain is less noticeable while getting dressed.

  • Neck pain is reduced while driving.

  • Lower-back pain begins later during sitting or walking.

  • Knee pain is less intense when standing from a chair.

  • A tight muscle becomes less tender and movement feels easier.

Treatment response and the number of visits needed vary depending on the cause of the pain, how long it has been present, the patient’s health, and whether the activity that aggravates the area is continuing.



The Body Has a Reason for Responding to Acupuncture


Acupuncture does not depend on one single mechanism.

It creates a mechanical stimulus in the tissue, activates sensory nerves, influences local pain-related signaling, and communicates with the brain’s pain-regulating networks. Research continues to study how these processes interact, but acupuncture is not simply a needle placed into a sore muscle. (PubMed)

침 치료는 근육통에 어떻게 작용할까요?


근육이 계속 뭉치고 아플 때는 쉬는 것만으로 충분하지 않은 경우가 있습니다.

아픈 부위를 만지면 단단하게 굳어 있거나 덩어리처럼 느껴지기도 하고, 움직임이 줄어들면서 주변 근육까지 함께 긴장할 수 있습니다. 이런 상태가 오래되면 다음과 같은 반복이 생길 수 있습니다.


통증 때문에 몸에 힘이 들어가고, 근육이 계속 긴장하면서 움직일 때 다시 통증이 심해지는 것입니다.


침 치료는 단순히 아픈 곳에 침을 놓는 치료가 아닙니다. 가느다란 침을 이용해 피부, 결합조직과 근육에 적절한 기계적 자극과 감각 자극을 전달합니다. 연구에서는 이러한 자극이 치료 부위의 조직 반응, 통증을 전달하는 감각신경과 우리 몸의 통증 조절 시스템에 영향을 줄 수 있다고 보고 있습니다. 여러 작용이 함께 일어나면서 통증을 줄이고 움직임을 조금 더 편하게 만드는 데 관여할 수 있습니다. (PubMed)



침은 근육과 주변 조직에 실제 자극을 전달합니다


침은 매우 가늘지만, 우리 몸은 침 자극을 그냥 지나치지 않습니다.

침이 조직 안으로 들어가고 적절하게 자극되면 근육을 둘러싸고 있는 결합조직과 상호작용합니다. 실험 연구에서는 침을 조작했을 때 콜라겐이 침 주변으로 감기고, 주변의 섬유아세포가 형태와 내부 구조를 변화시키는 반응이 관찰되었습니다.

이렇게 물리적인 자극이 세포 안의 생물학적 반응으로 전환되는 과정을 기계적 신호 전달, mechanotransduction이라고 합니다. 이 반응이 실제 치료 효과에 어느 정도 기여하는지는 계속 연구되고 있지만, 침 치료가 단순히 피부 표면만 자극하는 치료가 아니라는 점을 보여주는 연구입니다. (PubMed)

어깨 통증, 목 통증, 허리 통증이 있는 환자에게 침을 놓는 목적은 근육을 억지로 풀어버리는 것이 아닙니다. 아프고 긴장된 부위에 적절한 자극을 주고, 치료 후 근육의 긴장과 압통이 줄어드는지, 움직임이 편해지는지를 확인하는 것입니다.



침 치료는 통증 부위의 신호를 조절할 수 있습니다


침 자극은 치료 부위 주변의 화학적 신호에도 영향을 줄 수 있습니다.

동물실험에서는 침 치료 후 자극 부위에 아데노신이라는 물질이 증가하는 것이 관찰되었습니다. 아데노신은 신경의 활동과 통증 민감도를 조절하는 데 관여하는 물질입니다. 특히 아데노신 A1 수용체가 활성화되었을 때 염증성 통증과 신경병증성 통증 모델에서 통증 반응이 감소했습니다. (Nature)

물론 한 가지 물질만으로 침 치료의 모든 작용을 설명할 수는 없습니다. 통증은 그보다 훨씬 복잡하기 때문입니다.

하지만 침이 치료 부위의 감각신경에 전달되는 통증 신호를 조절할 수 있다는 생물학적인 설명 중 하나가 됩니다. 만졌을 때 근육이 심하게 아프거나, 움직일 때 통증이 생기거나, 반복적으로 근육이 굳는 환자에게 이런 작용이 도움이 될 가능성이 있습니다.



침 자극은 신경계를 통해 뇌에도 전달됩니다


근육에서 통증을 느끼더라도, 결국 통증을 해석하고 조절하는 것은 신경계입니다.

침으로 피부와 근육의 감각신경을 자극하면 그 신호가 척수와 뇌로 전달됩니다. 사람을 대상으로 한 영상 연구에서는 침 치료가 통증을 처리하는 뇌 영역과 뇌의 하행성 통증 조절 시스템에 영향을 줄 수 있는 것으로 나타났습니다.

만성 허리 통증 환자를 대상으로 한 연구에서는 4주간 실제 침 치료를 받은 그룹이 가짜 침 치료 그룹보다 통증으로 인한 불편함이 더 많이 줄었습니다. 또한 통증 조절, 감정과 동기부여에 관련된 뇌 영역들의 연결에도 변화가 나타났으며, 이러한 변화는 통증 불편감의 감소와 관련되어 있었습니다. (PubMed)

사람을 대상으로 한 PET 연구에서도 실제 침 치료와 가짜 침 치료가 뇌에 서로 다른 반응을 일으켰고, 우리 몸의 자연적인 통증 조절에 관여하는 뮤 오피오이드 수용체 시스템에도 차이가 관찰되었습니다. (PubMed)

쉽게 말하면 침 치료는 아픈 근육만 보는 것이 아니라, 너무 예민해져 있는 통증 신호를 신경계 차원에서도 조절하는 데 관여할 수 있습니다.



아픈 근육은 왜 계속 긴장할까요?


움직일 때 통증이 생기면 몸은 그 부위를 보호하기 위해 자연스럽게 힘을 줍니다.

어깨 통증이 있으면 팔을 들지 않으려고 하고, 허리 통증이 있으면 몸을 뻣뻣하게 움직이거나 다른 근육에 더 힘을 주게 됩니다. 목 통증이 있는 환자는 운전할 때 고개를 충분히 돌리지 않게 될 수 있습니다.

처음에는 몸을 보호하기 위한 반응이지만, 이런 움직임이 계속되면 근육의 긴장이 유지되고 정상적인 움직임이 더 어려워질 수 있습니다.

침 치료는 통증 민감도를 낮추고 아픈 부위를 조금 더 편하게 움직일 수 있도록 도와 이러한 통증과 긴장의 반복을 끊는 데 사용될 수 있습니다.

그래서 치료 경과를 볼 때 통증이 완전히 없어졌는지만 확인하지 않습니다.

  • 고개를 이전보다 편하게 돌릴 수 있는지

  • 어깨를 조금 더 높이 들 수 있는지

  • 허리 통증 없이 앉거나 걸을 수 있는 시간이 늘었는지

  • 의자에서 일어나는 것이 편해졌는지

  • 통증 때문에 잠에서 깨는 횟수가 줄었는지

  • 단단하게 굳었던 근육과 압통이 줄었는지

이러한 변화도 통증 숫자만큼 중요합니다.



목 통증과 어깨 통증 침 치료


목 통증과 어깨 통증은 함께 나타나는 경우가 많습니다.

허리 통증 침 치료

다만 모든 허리 통증이 근육 문제인 것은 아닙니다. 심한 근력 저하, 빠르게 악화되는 감각 이상, 대소변 조절 변화, 발열 또는 큰 외상 이후 발생한 통증은 신속한 의학적 검사가 필요합니다.



무릎 통증과 그 밖의 근골격계 통증


무릎 통증에도 근육은 중요한 영향을 줍니다.

무릎이 아프면 엉덩이, 허벅지와 종아리 주변의 근육이 긴장하거나 정상적으로 사용되지 않을 수 있습니다. 이로 인해 걷기, 계단 이용과 의자에서 일어나는 동작이 더 어려워질 수 있습니다.

  • 통증은 언제 시작되었는지

  • 어떤 활동과 움직임에서 심해지는지

  • 통증 때문에 잠을 깨는지

  • 뻣뻣함, 저림, 감각 이상 또는 근력 저하가 있는지

  • 얼마나 오래 앉거나 서거나 걸을 수 있는지

  • 이전에 영상검사, 주사, 수술, 물리치료 또는 다른 치료를 받았는지

  • 다시 편하게 하고 싶은 활동이 무엇인지

필요한 경우 통증 부위의 압통, 근육 긴장, 관절 가동 범위, 움직일 때의 통증과 양쪽 상태의 차이도 살펴봅니다.

상담과 평가를 바탕으로 치료 계획을 정합니다. 목 통증, 어깨 통증, 허리 통증과 무릎 통증은 환자마다 원인과 상태가 다르기 때문에 침을 놓는 위치와 치료 계획도 모두 같을 수는 없습니다.



침 치료가 도움이 되기 시작하면


치료 후 가장 먼저 아픈 근육이 조금 부드러워지거나 묵직한 느낌이 줄었다고 하는 환자도 있습니다. 다른 환자는 고개를 돌리거나 팔을 들거나 걷고 앉는 동작이 조금 편해졌다고 느낄 수 있습니다.

통증 때문에 잠에서 자주 깨던 환자는 수면의 변화를 먼저 느끼기도 합니다.

저희는 치료 후 잠깐 느낌이 달라졌다고 해서 문제가 해결되었다고 판단하지 않습니다. 일상생활에서 환자가 실제로 느낄 수 있는 변화를 확인합니다.

  • 운전할 때 목을 조금 더 편하게 돌릴 수 있는지

  • 단단하고 눌렀을 때 아팠던 근육이 덜 민감해졌는지

치료 반응과 필요한 치료 횟수는 통증의 원인, 지속 기간, 환자의 건강 상태와 통증을 악화시키는 활동이 계속되는지에 따라 달라집니다.
